All requests for appointments will have to be made in advance for appropriate location and an appointment date will be scheduled. We will try our best to ensure an expedient scheduling of appointment date and that all patients are seen promptly.

Yes, you will need a valid referral from your GP or another specialist. A referral is needed to be eligible for Medicare benefits for consultations and Medicare and Private Health benefits for procedures.

We request you to submit your referral to the office before your appointment so that Dr Kumar can review your information ahead of time.

Dr Kumar values the role of your primary GP in care of your overall health and prides himself on a close relationship and excellent communication with your referring GP for best ongoing care.

• New patient registration form – Will be sent to you via email at the time of booking an appointment or can be filled online. You can fax it or email us back or bring on the day of appointment
